How to Get Started
New Patients — Your First Office Visit
Upon entering the office, our receptionist will greet you and assist you in completing the initial Patient Information form. If you have previously downloaded and completed this form, the receptionist will collect it at this time. If not, you will be given time to complete it before seeing the therapist or doctor. This paperwork provides us with information about your general health and your specific condition.

Next you will have a consultation to discuss your condition and concerns. If it is appropriate the therapist or doctor will recommend a thorough history and examination likely including:
- A review of medications, supplements, family history of illness.
- Review of all symptoms.
- Affects on sleep, work, activities of daily living, social and recreational activities.
- Previous testing (including reviews of reports.) Previous treatment. Previous accidents, injuries, hospitalizations and surgeries.
Following the history a specific physical examination most likely will be recommended, including procedures such as:
- Spine Mobility Evaluation
- Balance Testing
- Othorpedic Testing
- Postural Evaluation
- Muscle Strength Testing
- Vitals
- Neurologic Testing
- Review of X-rays/CT or MRI Report
- Joint Evaluation
If additional testing such as x-rays, MRI/CT or electrodiagnostic evaluation is indicated they would be recommended.
